If your insurance company requires a referral, please make sure you have contacted your primary care provider prior to scheduling to obtain your referral. Make sure to bring your current insurance card with you to each visit. Make sure to bring your current insurance card with you to each visit. If we are contracted with your insurance company, we will file your claim as long as we were able to confirm your coverage. Copayment, coinsurance, and all outstanding balances are collected at the time of service. We collect all out-of-pocket expenses such as deductible when scheduling for procedures and therefore payment is expected at the time of scheduling. Payment for services is the responsibility of the parent or legal guardian of the patient. We accept cash, check, Visa, Mastercard, American Express, Discover, Apple pay, or Android pay.

We have contracts with some, but not all, insurance companies. If we do, you will be responsible for any deductibles or co-payments at the time services are rendered. Your insurance policy is a contract between you and your company; we are not a party to that contract unless we also have a contract with your company. If your insurance company has not paid on your claim within 45 days, you will automatically be responsible for the balance. Please be aware that some, and rarely all, of the services provided to you may be deemed non-covered services or not medically necessary under Medicare and/or other medical insurance programs.

* We are not contracted with most of the plans offered through the Affordable Health Care Act. Please contact your insurance company to confirm coverage. If you have an insurance company that we are not contracted with or we have not been able to confirm your coverage, payment in full is required at the date of service. A copy of your charges will be given to you so that you may file yourself for reimbursement. We always recommend contacting your insurance company if you have any questions regarding our participation with your insurance company, your benefits, your out-of-pocket expenses, or if a referral is required.

Cosmetic Services

Any procedures considered to be cosmetic are not covered by insurance, and therefore payment is expected at the time of scheduling. We will not file any cosmetic service to insurance. We will notify you if we believe a planned procedure is cosmetic and will discuss costs prior to the procedure.
